Forms can be overwhelming for both staff and volunteers. So take the time to familiarize yourself with the following information.

The volunteer orientation checklist was developed to keep your paperwork and requirements in order and accountable. It is suggested to keep this form in the volunteers' file.

Use the following forms for NEW 4-H volunteers wishing to sign up and join the Maryland 4-H Program.

Phone or Face to Face Interview Form

Some volunteers may only be needed on a short term basis, possibly for an event or program. The following form can be used in place of MEP 316.

Short Term Application

This letter can be used when informing a volunteer of their acceptance or rejection into the 4-H volunteer program.

Volunteer Acceptance / Rejection Letter

Yearly updates and evaluations should be conducted for current and contining volunteers. The following forms should be completed.
